Eligibility. Applicants must be 18 years of age to enter. Entrants must be a resident of Arkansas. No purchase necessary to enter. No employee, spouse, agent, partner, member or affiliate of Playtime Pizza, Crain Automotive Group or Crain Media is eligible to win.   Limit one entry per person per day. Entrant does not need to be present at drawing to win.

 

How to Enter. Customers of Playtime Pizza will receive an entry form upon admittance into the Playtime Pizza restaurant and gaming area, which can then be completed and submitted at Playtime Pizza. Anyone else who wishes to enter the promotion can make a request for an official entry form either (1) in person at Playtime Pizza or (2) by mailing a request, along with a self-addressed, stamped envelope, to Playtime Pizza Corporate Office, 135 Southtowne Blvd, Hollister, Mo 65672. An official giveaway entry form will be mailed within one (1) business day of the request (i.e. within one (1) day of an “in person” request and within one (1) day of actual receipt if request sent by mail).

 

Random Drawing.  Recipients of the twelve (12) gifts will be selected in random drawings from all eligible entries submitted. Drawings will take place at 10AM CST beginning 12/11/09 and continuing daily for twelve (12) consecutive days. Attempts will be made to notify the winners via telephone using the contact information on the entry form immediately after each of the drawings. If live contact with the winner cannot be achieved by 2PM CST of the drawing date, an alternate winner will be selected. Winners must show identification and proof of address in order to redeem prize. All information must match the winning entry form.

 

Prizes. The winners are responsible for all applicable state and federal taxes (including income and sales taxes), title and license fees upon winning. Prizes are not redeemable for cash or alternate merchandise. All prizes are awarded “as-is”. Playtime Pizza, Crain Automotive Group and Crain Media waive any express or implied warranties that any prize may have. Any manufacturer’s warranties, if applicable, will remain intact.

 

Release.   By participating in the giveaway, participants agree to (1) be bound by all decisions of Playtime Pizza relating to the giveaway, and (2) release and hold harmless, and agree not to institute any claim against, Playtime Pizza, LLC, Crain Media, Crain Automotive Group and any of their partners, members, employees, agents, affiliates or representatives from any and all injuries, losses or damages of any kind arising from or in connection with, directly or indirectly, the giveaway (or related activities) and/or the awarding, possession, receipt, use and/or misuse of the prizes awarded. Under no circumstances will any entrant be entitled to obtain awards for, and each entrant hereby waives all rights to claim, punitive, incidental, or consequential damages, or any other damages, including attorneys fees.

Name and Likeness.   By participating in the giveaway, participants grant Playtime Pizza and Crain Media permission to use his or her name and likeness for advertising and promotional purposes without any additional compensation (except where prohibited by law). By entering giveaway, entrants shall be deemed to have agreed to have their entry information made available to the Playtime Pizza and Crain Media. Drawing winners must be available to come to Playtime Pizza, 600 Colonel Glenn Plaza, Little Rock, AR for the finale on December 23, 2009.
